CEO Message: Transformation Update, August 5, 2024

Transformation Updates at a Glance:  

  • Launching "Pulse Check", a new initiative to monitor and drive transformation progress against Transformation Roadmap. 
  •  Ongoing engagement with ELT to explore portfolios and best practices for health organizations. 
  • Drafted current state recruitment process based on various inputs and identified opportunities for improvement.

Hi everyone, 

As we continue our journey to transform our healthcare delivery, I want to provide you with this week's update on our continued progress and plans for the upcoming week.  

The Transformation Office has updated the Transformation Communications Plan to broaden our reach and create more opportunities for feedback. Our goal is to ensure every stakeholder feels heard and valued in this process. We’ve also developed an integrated Transformation Roadmap which will guide our collective journey over the coming months. The TO will be launching a new initiative called “Pulse Check” designed to provide a structured and collaborative approach to monitoring and driving progress of key transformation milestones.

The Organizational Capacity team has continued to meet with members of ELT to understand their portfolios, challenges, and opportunities. The team is conducting a jurisdictional scan regarding best practices for high-performing health organizations, leveraging these learnings, and identifying themes for further discussion during their upcoming workshop. This process is designed to enhance our organization's effectiveness and create opportunities for growth and development, not to eliminate employment. 

The Workforce Recruitment team has conducted an initial workshop and 11 one-on-one interviews with DHW, PSC, Talent Acquisition, Medical Affairs and HR regarding the existing recruitment processes for health care providers, support staff, and management. This information has been leveraged by the team to document a draft current state process, deepening the understanding of the workforce needs and challenges. Going forward, the team will focus on the future state of workforce recruitment. This work will start with some short-term improvement opportunities.  

The Patient Medical Home Operating Model workstream has been particularly engaged this past week, conducting several site visits to observe challenges and opportunities firsthand. Additionally, the team has connected with the Quality Improvement team to explore ways to advance pinpointed improvement opportunities. Going forward, the team will collaborate with primary care managers to confirm the streamlined approach for activities required to establish or expand PMHs, thereby informing the tactical end-to-end roadmap and recruitment plan. 

The Patient Registry workstream team has completed its initial review of the registry data and identified several opportunities to improve data management and remove names that do not belong on the registry. The team will continue to collaborate closely with partners, including primary care, to identify the most appropriate approaches to confirming affiliation and attaching unaffiliated patients to PMHs.  

The Medical School Learning Support team has conducted meetings with new stakeholders, broadening our network of support and expertise regarding HPEI’s medical education program and professional practice. The team is continuing to review the current infrastructure, medical education program, etc., and is setting up a recurring HealthPEI / UPEI working group to drive collaboration and streamlined decision-making. They are also preparing for an upcoming health learning system visioning session.
